Citizen Service Request Form

This link allows residents of Carlinville to request service for non-emergency service requests or notify the city of any code enforcement notifications for all city departments. For issues that need immediate attention, please see phone numbers below:

911- For all life safety/medical/Emergency Response/Public Safety Issues

217-854-3221- Police Department Non-emergency line (Public Works After Hours Phone Number 4:30 PM- 8:30 AM and holidays)

217-854-4076- Public Works Office (8:30 AM- 4:30 PM- Monday thru Friday)

You will need to complete a separate request for each service request or issue that you are reporting. This allows us to route to the proper department and track. When submitting a request for service or issue, please provide the following information in the boxes below:

  • The exact street address where the problem or issues are located at
  • Provide as much detail as possible to allow us to understand the issues or request
  • Contact information, so that we can contact you if we need additional information
